but thereafter subject to change without notice.

The goods and services shown below are the most commonly utilized by our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

PROFESSIONAL SERVICES

Basic Services of Funeral Director and Staff ..................................................................................................................$1,900

Our fee for the basic services of Funeral Director and Staff includes, but is not limited to:

• 24 hours per day/365 days per year availability of a funeral director and/or staff members to respond to initial request for services, arrangement conference with family or responsible party, and coordinating the arrangements for the service, memorial service or gathering.
• Staff to advise, assist and recommend appropriate personalization or individualization of the services to commemorate the unique life of you loved one.
• Clerical assistance in the completion of various forms, such as Social Security, Veterans Benefits and statistical data.
• Obtaining and recording biographical information and statistical data.
• Preparation and transmission of obituary notice/editorial tribute in newspaper(s).
• Death Certificate preparation, filing and securing the necessary permits, authorizations, consents and assignments.
• Initial caretaking and sheltering of the deceased for the first 24 hours.
• Coordination with those providing other services such as hospital, nursing facility, assisted living, home/hospice facility, doctor’s office, medical examiner, cemetery, vault company, our crematory, clergy/celebrant, organist, pianist, soloist, and any others involved in the services and final disposition.
• Administrative services, insurance, facility maintenance, utilities expenses, taxes, legal and accounting fees, record keeping, equipment and inventory expenses, and government compliance (OSHA, ADA, etc.).

This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for immediate burial, direct cremation and the forwarding or receiving of remains.)

CARE OF YOUR LOVED ONE

We require the use of licensed personnel to supervise all technical aspects of the services provided to those loved ones placed in our care. The utmost “attention to detail” and respectful care is taken in all steps of the preparation process. Our facility is OSHA and state compliant. Our personnel are dedicated to providing the best possible experience to each family that we serve.

Embalming is a chemical process that provides temporary preservation of the body and eliminates certain health hazards. Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

Our charge for an immediate cremation, without any attendant rites or ceremonies, includes removal and shelter of remains, local transportation to the crematory,
necessary services of funeral director and staff, and authorizations. If you wish to arrange a direct cremation, you can use an alternative container.
Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering).
The containers we provide are made of cardboard.
